Certificate Request Form
Some certificates are now available through e-mail. If the certificate you are requesting is available electronically, the e-mail column on this request will be enabled with a check box. If you check this box, the certificate will be forwarded to the e-mail address entered on this request. If you do not check the box, the certificate will be delivered through regular mail.

The default method of sending certificates is by regular mail unless the e-mail box is checked. If you do not see an e-mail box, or have trouble ordering certificates for e-mail delivery, contact 617-727-8451 for assistance.
